Rarely. The odds are that it will not act like such a good augmentor. The nortriptyline can be taken at bedtime. It might help you sleep in place of the trazodone.

Effexor. Maybe Lexapro or Cymbalta.
Cymbalta might help with pain more than Effexor.

It isn't guaranteed, but it did put a lot of weight on me. I would recommend that you start practicing now to change your eating behaviors. Eat small meals every 3-5 hours and wait 20 minutes to decide whether you are still hungry.
The usual target dosage is 75mg. I take 150mg. Start no higher than 25mg for the first few days. It can be quite soporific (excessive sleep promoting) at first. Assaying blood levels works well with nortriptyline. The effective range is between 50 and 150 ng/ml (supposedly). You still have to titrate the dosage clinically (empiracally), using the blood level as a rough guideline.
