If you have definite or probable Borrelia infection, you should be treated with antibiotics. Usual first-line choices for early infection (eg. erythema migrans) include amoxicillin and doxycycline. Ceftriaxone injection is suitable for seriously ill pts with Lyme meningitis, cardiac infection, and other severe manifestations. Less severe neurological manifestations can be treated with 'double dose' oral doxycycline, continued for several weeks at least. It is even possible to treat most milder case of Lyme meningitis with doxycycline 200mg twice a day (with food). Non-neurological Lyme is treated with 100mg twice a day.
Some people diagnosed in the US with chronic Lyme disease haven't ever had clear evidence of Borrelia infection, and indeed may not need antibiotics... this is a very controversial area.
