Why doesnt every patient on an MAOI take an NRI?

NRIs such as Nortriptyline & Reboxetine block the tyramine reaction with MAOIs such as Nardil & Parnate.
Isn't it logical to add the NRI to an MAOI, to increase its efficiency and safety?

I am currently on Nortriptyline 75mg, and looking to add Parnate.
