I haven't been posting here for quite some time (years), but now I stumbled upon something I can't put my finger on..
I have been using clomipramine for 14 years now, mainly for panic and ocd.
Since 2009 I occasionally experience orthostatic hypotension from sleeping too long and then getting up. Heat is an issue as well. The hypotension then is persistent for a whole day.
What strikes me is that when this is happening, my ocd is actually worse and so is my overall mood, I almost feel dysphoric. Alongside the usual symptoms of hypotension such as dizzyness, faintness, tingling in fingers etc, I also experience extreme irritability.
I know the orthostatic hypotension originates from clomipramine's alpha adrenergic receptor blocking effect, but what is the relationship with the therapeutic effectiveness of the drug ? Can anyone elaborate on this ? Is this common with other antidepressants as well ?
