Hi. Sorry you're having these problems. I can only guess at what might me going on and what might help. Firstly, sounds to me that strong NARIs/Noradrenergic meds are a big NO for you. And as you respond better to methylphenidate rather then amphetamines shows that you clearly have focus/attention span issues. When I've taken concerta/ritalin I feel speedy but without the phisycal effects. And I feel smarter too! But, like some nootropics (smart drugs) such as DMAE they can worsen the down stage of bipolar. My guess is that dopamine helps, but anything that raises dopamine, the 'reward' chemical is either hard to get on or is a controlled substance. Because we might just get too 'happy' from it. Poor old amineptine :( So you've tried quetiapine? Small doses enhance other antidepressants while higher doses around the 600mg mark can do wonders for bipolar.
One Nootropic that I am currently experimenting with is centrophenoxine. Makes me think clearer and I feel far more awake. Its allowed me to cut out caffein too. It has one brand name- Lucidril which may or may not be available on prescription. I'm in UK and order it legally online. Ever tried modafinil? a mild psychostim that enhances wakefulness, concentration and mood, tho some (like myself) build tolerance fast. There are lots of nootropics you can choose from. Try to research them the best you can. They are 'smart' drugs. And I'm sure a good number of them would help your ADD improving focus, mental stamina, wakefulness and memory.
Also there are new meds coming out all the time. One of them works on the opoid receptors. Look above for the long posts. I see my doc Friday and my mixture is doing didly for me right now. I'm going to ask to swap my amitriptyline for tianeptine, the reuptake enhancer.
