Compounding pharmacies make it, but you do need a prescription for them.
The other option would be to get a doctor to prescribe one pill (50mg), or order the smallest quanitity you can internet, and customize your own doses. With doses being 1.5mg to start, that would mean somehow getting 40 doses out of one pill. If I were doing that myself I would mix it with rice flour to get a big pile, and then it would be easier to divide into small piles. The inherent inaccuracy in that method would not make much of a difference. Some people take it every other day, with varying doses from .1mg to 1.5mg, so variation at such a low dose won't be a big factor.
Best bet, find as much on the worldwide web as you can on LDN, print it out, and bring it to your doctor. Along with a phone number of your chosen pharmacy (listed at the LDN home page). There is no risk and nothing to lose when you're taking 1/50th of a pill like Naltrexone.
I have found 1.5mg is too much for me. Even .75mg feels too much because after about 3 days of accumulation it feels like the opioid receptors are getting blocked too much to take advantage of the increased opioids floating around. Every other day, or noon dosing, or smaller dosing, are things I need to experiment with.
