If you had childhood trauma and/or - repressed memories or emotional repression despite intellectually processing the events, I think you should give psychoanalysis/psychodynamic therapy a shot if you haven't yet tried it.
I had more insight from just one day of this type of therapy than 8 years (though intermittent) of "talk therapy" and/or CBT. As for my symptoms, I felt anxiety emerge, then turn into grief and then just disappear. That's just one day, but think of that happening cumulatively over time.
It might be difficult to find a skilled psychiatrist who does this, but it may be worth the effort.
Don't give up.
