You have to be sober for at least a week before you can start. It prevents physical cravings for alcohol, and anxiety associated with it. It does not cause aversion to alcohol. It can cause stomach upset when it's taken at first, though that subsides.
It should be supported with intense counseling - medication alone is probably not enough to keep someone sober for any length of time.
I took it for a year and a half. In a few weeks I'll be celebrating three years of continuous sobriety. I don't think I would have gotten through the first six months without Campral.
