I'm probably not representative of the general population, since I feel some effect of ridiculously low doses of lithium. But here's my story.
For lithium carbonate at a dose of 112.5 per day (a 450 mg tab quartered) is enough to keep the worst of my mixed states in check. But even at that dose I get a strong urinary tract (UT) reaction. (The feeling like I have to go #1 all the time; it seems to be due to irritation.)
I recently tried switching out in favor of lithium orotate. I think that 65ish milligrams is doing about the same for me, although I might need around 90 mg. So that's about a 1/2 or 3/4 to 1 equivalency (LiOr to LiCO3).
The orotate does seem to reduce the irritation in my bladder. Also, my vision has gotten decidedly less blurry. So those are improvements.
BUT, on the orotate my kidneys end up hurting instead, which I don't think is a good thing. I may have to go back to the carbonate (or the citrate).
Then again, I seem to have an incredibly sensitive system... If you've not been having UT problems on the carbonate, you may very well not experience any issues with the orotate. (Although, do keep in mind that some people end up not derived much benefit from the orotate.)
