When I was hospitalized, my meds got switched around...my trileptal was lowered, my antidepressant was raised, my buspar was increased, and I was given a kind of hefty dose of hydroxyzine (200mgs/nightly) in place of as-needed Trazadone. When I visited my normal shrink afterwards, she was not pleased...she was downright pissy, and asked me things like "why exactly did dr.___ lower your trileptal? Is there any reason for increasing your antidepressant this much?" and "I personally don't like to prescribe hydroxyzine at this dosage..did he explain this to you?" I felt like I was caught in the middle of some sort of contest in which they both felt like they had better answers than each other. *sigh* And I have friends who would be on say, Straterra, and go to a doc who'd RX Ritalin or Adderall, saying "I simply don't think Straterra is effective enough". But...other friends would go to a shrink when already RX'd Ritalin or Adderall and the new doc would want to switch to Straterra *immediately* because of "addictive potential," even if they'd been doing just fine for years. And then there are more subtle changes...Klonopin to Tranxene, Ambien to Sonata, Effexor to Cymbalta, that happen all pretty frequently b/c a doc has a personal preference.
