If your history indicates that hypomania escalates enough to adversely affect your life, or if it ultimately leads to a subsequent depression, I would recommend that you treat the mania as soon as possible by taking Zyprexa or Depakote temporarily to maintain stability. Ideally, you would not want to alter your Cymbalta regime. I doubt taking it every other day would be of much help. It might even cause the mania to accelerate through "pulsing" the drug. It would be better to just lower the daily dose.I hope you don't have to interrupt your taking of Cymbalta. I have seen to recent failures of Topamax to prevent hypomania. I don't think it makes such a good anti-manic agent or monotherapy mood stabilizer. It might confer stability further down the road as a prophylactic, but I don't think the drug has really proven itself yet. Still, some people here really do like it.
Sooo. If you don't have any antipsychotics or Depakote to work with, and you are concerned with the escalation of mania, I guess I would increase the Topamax and lower the Cymbalta temporarily. I can't think of anything else right now that makes sense except that you also try to keep a regular sleep schedule.
