I found that Mylan's lorazepam, the generic for Wyeth's Ativan, was so inferior that it didn't even seem to be the same drug. (It acted like a sleeping pill.)
I gladly pay the 6x higher copay to get the name brand. (Which acts like an anxiolytic.)
If your insurance company has "tiers" you may have the option to simply ask for name brand, and pay a higher copay.
