It is hard to tell via this mode of communication, obviously, but it sounds like it might be tics. Does he have any vocal tics like sniffing, grunting, throat clearing, barking, swearing, whooping? Even if not, it is very common for motor tics to emerge before vocal tics in Tourette Syndrome.

It could be med related, with or without the movement disorder. Sometimes kids with Tourettes, with only mild and barely noticeable symptoms, start having more overt tics while on stimulant or other meds.

Consult a pediatric neurologist.
