I yend to think of it as a good thing when a diagnosis made, so that you can finally get treatment. This is a lot better than having mysterious symptoms that have no explanation.

what test did they do exactly? find living lyme infection in the brain water or just antibodies?

A positive test is awesome! The test is frequently inaccurate. To get a positive is actually a blessing, to actually know where the psych symptoms are coming from. How about cork screw shaped creatures drilling holes in your brain? That's what Lyme is.
The whole reason I ever became a psych patient in the first place is because of this statement from a doctor, "I'm happy to tell you your Lyme test is negative and you don't have Lyme disease....here is a recommendation for a psychiatrist for your depression".
That led to a 20+ year nightmare, suicidal, failed ECT, failed drugs, treatment resistant depression, anxiety, even some mild psychosis at times.
Eventually many years later - long story - it became evident that maybe I had Lyme instead of depression. That journey led to remission 3 years later.
All that suffering was caused by a doctor claiming I did not have Lyme.
Jereon you need 2 to 3 antibiotics at the same time for many months to get better. I doubt they will do that. I hope they do. In USA only LLMDs do that (Lyme Literate MDs). But you can get similar results with the right herbs. Get herbal books on Lyme and study them. Or if you can order some, ask, I will point you in the right direction, names, doses, everything.

Doxy is the most common med in Lyme. Not likely to bring remission by itself, but significant improvement I would expect. Likely will take months not weeks, for other people maybe 9 to 12 months, for me it was 36 months.
Double the max dose is best. Shoot for 400mg to 500mg per day. That's because lower doses do not penetrate the blood brain barrier and lower doses do not actually outright kill the bacteria. Lower doses weaken the bacteria. You want to nuke them. High doses almost mandatory.
Do daily doses in single doses - not twice a day or 3 times a day. The reason is so that you get a huge massive blood spike each day, and a trough as well. It's sort of a "pulsing" strategy, where you purposely avoid a steady blood level. Bugs go into hiding with steady blood levels, wasted antibiotics.
Either online or in books, read about herbs to help with Lyme. See if you can choose a couple and get your hands on them. At the very least, on days you don't have much interaction with other people, eat a whole clove of raw uncooked unheated garlic - extremely potent against lyme - but extremely smelly too. :-) Eat a clove a garlic toward the end of a meal, not the beginning of a meal, so it metabolizes correctly. Garlic supplements, or cooked garlic, are useless against lyme. The healing chemical is actually the cause of the odor. No odor, no healing.
The very first thing I ever took for lyme was raw garlic. To be honest with you, it was amazing. Felt unusually peaceful, less pain, more energy, more interest, more hope. But wow did I stink! :-) Took about 3 days for good feelings to start. By then I was really stinky. I worked in a retail store so I could not keep doing that.

Easy answer. You cannot be sure. Docs told me that many times when patients reach remission, we never know for sure what the organism was - mutating all the time, new versions all the time, at least half a dozen other co-infections just as bad as the main infection.
It gets diagnosed wrong very often because it is a hard diagnosis to make. Lyme is the Great Deceiver. It looks like depression. Looks like schizophrenia. Anxiety. Looks like arthritis. Looks like Fibromyalgia. Chronic Fatigue Syndrome. Is even misdiagnosed as MS or Lupus. One patient may have 30+ symptoms while another feels fine except for unexplained nausea. Symptoms are all over the map from one person to the next. If you look at a full description of major depressive disorder, it fits perfectly with lyme as well...the mood thing, sleep thing, eating thing, sluggish thing, on and on...looks the same. If someone has been on psychiatric medicines for 10 years or more and not much improvement, if any, that is a big red flag to suspect infection.
It's a clinical diagnosis. Combination of clusters of symptoms, lab tests, experience, and instincts. The only lab test with decent accuracy though still makes mistakes is Western Blot by either Stonybrook University or IGenex Lab. Other labs doing the same test are not as reliable.
Vitamin D is almost always very low. It's an easy accurate lab test to find out. This test alone could be used to screen potential lyme patients.
CD57 is an immune marker, can be lab tested easily, which is often low in lyme.
The only solid diagnosis is to actually start treatment on a suspicion to see what happens. Distinct response patterns are diagnostic. For example commonly what happens is, very quickly with treatment, patient will say they feel better than they have felt in a long time - but it's a short honeymoon and soon ends in a crash worse than where started.
Once in a while we get lucky and the lab test actually comes back positive. The tests do work sometimes.
I was pretty sure a family member had lyme. They tried antibiotics for a couple weeks as a test. There was no reaction at all. No good reaction, no bad reaction, no nothing. That pretty much meant she doesn't have lyme. We've since narrowed it down to sugar issues and/or yeast issues. Possibly mercury issues from amalgams but we replaced those and think we got most of it cleaned out with alpha lipoic acid.
