My AP regimen is:
Zyprexa 5 - 0 - 7.5 -0
Seroquel 0 - 0 - 0 - 500
Seroquel ER 0 - 0 - 100- 0What should i change? I usually get paranoid at around 5-6 pm and it stops after i take my nighttime dose of Seroquel.
Take some Seroquel ER at noon?

Can you split up your Zyprexa just for a day or two and take a dose when you're feeling paranoid? Might be worth the experiment.

I think you already tried Saphris (asenapine), but I thought it was worth mentioning that Saphris is particularly effective for treating paranoia.

actually no i havent. Its on my list though.
Germany doesnt have it approved for schizophrenia for some reason. But its available.

Cut the 7.5 in half, one morning and noon then 5 in the evening.
Or
Switch to all xr, take one morning, one evening.
Or do both. Might be best.
I take 3 doses of meds a day. 6 hours apart. 8mg perphenazine x3. And some dose of zyprexa x3. Dose verys depending on how I feel. Or how tired. Or racey.

I'd add some Haldol, 5 mg.

haldol is an emergency med even more so than Zyprexa and it gives me bad akathisia.
I cant understand how you can recommend this to somebody who is neither at risk of harming himself nor others.

I recommend it because it is one of the most effective agents against paranoid thinking. Haldol in small doses can be very tolerable, but EPS can be a major complaint.
Haldol is not just for 'somebody' who is at risk of harming themselves or others. There is a misconception that Haldol should on only be used in emergencies, when many psychiatrists prescribe a low dose for maintenance. I was prescribed Haldol 5 mg daily for paranoid thinking and it absolved my thoughts. I'm lucky that I tolerated it well.
