Hello, someone once told me eons ago that the rages I have emanate from the temporal lobes, and I'm wondering about that since. I have constant states of extreme irritability, agitation, and destructive aggression that results in destroying things. Along with severe depression. Risperidone helps but not working so well since some time. I'm wondering that since aggression is the primary symptom and it's constant, whether I might have temporal lobe dysfunction. I want to know if that's possible without there being any seizure-like symptoms or epilepsy and normal EEGs. Can one have tld without seizure or epilepsy? I have no tbi and a normal CAT scan. I'm currently diagnosed with Impulse Control Disorder, MDD and Borderline, previously schizoaffective. But I feel as if my disorder is so many volumes more, it's extreme! and aggression is the primary psychopathology, not psychosis which I haven't experienced but the aggression is very irrational and un/minimally-provoked.

Hi when you say destructive rages who do you take it out on? Do you smash things? Fight with others? How long does this state last? Phillipa

Phillipa, I destroy things, smash glass objects, once I destroyed a CRT and destroyed and banged a lot of furniture which lasted for two days until I was medicated with risperidone again. I even threw a pair of iron scissors on the floor (very dangerous) and could have hurt someone, and proceeded to smash and demolish an electric iron, hurting myself in the process as the plug flew on my head and my head started bleeding. I really cannot control myself, though I know what it is that I'm doing.. Smashing objects can last for half-an-hour, even an hour, but no matter what, when not on drug, I can't stay calm for no amount of time, but always in a state of agitation and depression in my head, and always with hyperirritability, cannot stand even for my mother to talk to me, shout her down.
But for past few years, when not on drug, I've taken to shouting at people, I remember past conflicts, with neighbours for example, and I shout lots of curses and shout at them outside their home not to harass my mother. (Which they have in the past, but not a major issue at that particular time. Though I feel as if everyone is antagonistic towards me and my family and I can't do anything about it.) A week ago, even though I've been taking risperidone 3mg x2 constantly since some time, I raged with irritability all the time, and when I heard a vulgar song playing in the roadside (I live in a pagan country, so that's common. I myself believe in Christianity), I started shouting at people (no one /specifically/), but just standing there and shouting at people about how there gods were loose and they were vulgar and many curses about them, curses that are very vulgar and they usually use, in a fit of rage. And for the ten minutes that I shouted before I regained my mind, I almost didn't know what I was doing.
And when not medicated, I experience a severe personality change, I'm so angry that I even call family members using swear words.

I honestly don't know what to say and wouldn't even attempt to try to formulate a diagnosis. Very complicated. I lean toward Schizo ones though. Phillipa

Most psych drugs are prescribed to people with (relatively) normal brains. The idea isn't to correct weirdness (that's what neurologists do); the idea is to alter brain chemistry to bring about changes in mood and behavior.
Have you tried Trileptal, Tegretol, Depakote? I would think (I'm not expert, just throwing stuff out there for you) that an anticonvulsant might be good. Also, if you still need an antipsychotic, taking an anticonvulsant might help you lower the antipsychotic dose, which is always a good thing.
I have Bipolar I (supposedly). When I had a different shrink, they diagnosed all kinds of personality disorders and all that, and they prescribed Trileptal, which I still take. Its easier to tolerate than Depakote and there's less concern about drug-drug interactions. Back then, I took 600mgs/day; now its 1200mgs/day. Back then, it was for "anger management;" now its used as my "mood stabilizer."
See what I'm saying? A lot of times, your diagnoses reflect your doc's opinions more than your actual problems. Factors such as gender, age, race, and social class play a role in diagnosis, too, at least at the aggregate level.
Blah blah blah...its always a good idea to go to neurologist, if you can. But even if your brain is normal, you may need/want meds to help you stay on an even keel. You're not the only one. When it comes to brain and behavior, a lot of is as yet unknown.
If you have the resource and inclination, I'd recommend an alternative doc, like a naturopath or an alternative-minded MD/DO. A more tolerable, cheaper way to bring these sorts of problems under control might be to use supplements, possibly in addition to any prescribed medication(s). Some people find that they can reduce dosages and/or taper off med(s) as time goes on while on an appropriate supplement program.

This behaviore is not typical of temporal lobe problems. Its more typical of frontal lobe issues. You could be bipolar and your mixed episodes may manifest as rage attacks-bipolar disorder involves a degree of disregulation in the pre frontal cortex. If your EEGs are normal and you haven't experienced a tbi, it would be hard to say what your affliction is.
One thing I would try is Lithium. Lithium has been used a long time to control aggressive behavior and if you do well on it we may have a clue as to whether your bipolar or not.

I also wonder if I have TLE (temporal lobe epilepsy) because I have intense deja-vu and it can be quite disturbing. along with many other symptoms that are listed as TLE
I too have rages and break things. Not as severe as you.Drs. like to give the schizo label too quickly. If you're not having auditory and/or visual hallucinations I wouldn't take the AP's

Hyperreligiosity sounds like one of your symptoms, also a symptom of TLE.
The treatment for TLE is Trileptal.
