Stopping sertaline, Starting mirtazapine

Decided this is going to have to bve the next step, as per the psychiatrists recomendation.

I'll keep taking the reboxetine for the depression, because it has been so helpful, and replace sertaline with mirtazapine in the hope that it is better for anxiety.

Also going to ask about a switch from alprazolam to clonazepam.

If the mirtazapine doesnt work, next stop (and last resort) is Nardil

Combining a NRI with Remeron may be more effective in the treatment of depression than its infrequent use would suggest. I like the idea. I am still not a big fan of reboxetine, though. It precipitated a suicidal anxious depression in me. If anxiety persists for you after adding Remeron, you might consider switching from reboxetine to nortriptyline.

I think I'm going to have to be going for a 3 day stay at the CSU (crisis stabilization unit) again. I have an appointment with a new psychiatrist april 11, but that's too far away when I can see a pdoc at the CSU the 1st day there and get Mirtazapine. I'm just feeling sooo anxious/empty/disconnected. I want a new brain grown with stem cells is what I want.

Anyway so I guess we'll be starting Mirtazapine around the same time, unless the pdoc there can convince me of a better idea.

God, how I long for the days when my medicine chest consisted of a bottle of ibuprofen, a box of Sudafed and some band-aids - now it looks like it could supply the health needs of a small african country.

Scott, re the reboxetine and nortriptyline, i know that theoreticaly nortriptyline should be a better bet for anxiety (and that article in the BMJ gave reboxetine a very bad rap) but for me they have been about equal, both very helpfull for depression, notr very helpful for anxiety (and as for all of the SSRI drugs, of no apparent use for anytyhing)

FWIW, mirtazapine has never helped my anxiety. Have you tried fluvoxamine for anxiety?

I will be interested to know how you find the hangover next day...I could not tolerate it and felt kind of poisoned until 3pm. It was neither calming nor the reverse. There were a few threads here about Remeron Rage. I loved it while I was asleep.

if mirtazapine doesnt work, i have decided that Nardil is the next stop (indeed, i think I'm like to jump straight to Nardil because of its GABA activity, and take it along with my reboxetine.)

Anyway, seeing the doc this afternoon - see what he has to say and what he thinks we should go with.

Australian doctors often dont want to come and work in Darwin (its 3000km from the bearest caopital city and department store and is thus very isolated, so we often have to see doctors from third world countries who come here as guest workers, but i am lucky enough to have found a doc who trained ata british university, and who is a member of the royal college of physicians.

He's smart, compassionate, and always includes me in treatment decisions 9and he isnt terrified of prescribihg Xanax - he's cautions, and I know when I ask for a script he calculates how many days since my last one to make sure I'm not exceding the stated dose - i feel lucky to have found him)
