Usualy they work great no problems. But once in a while they seem to do nothing. Thats why my doc gave me some 5mg zyprexa's to take in this situation. Last week it happened again, the zyprexa and perphenazine did not work for 3 days. I can tell, because I was only sleeping few hours a night and being tired and nonfunctonal at work. Forgetting things. At first I thought it was because of the decrease in perphenazine. But it had not been a problem for the last month that I had decreased. I called my doc on thursday out of pure desperation to put me back up on the meds. But when she fianaly got a hold of me I had taken a 5mg zyprexa and fell right to sleep. And felt instantly better at work all day. Even thought I was late for work because I was so tired. Anyways the meds are working great again, ever since the extra pill. Now I take the 15mg zyprexa split into 3 times a day. So far no problems. I realy just don't understand when they stop working for a couple of days. As far at the extra 5mgs, I made up for it by taking less later, and I was fine.

Maybe its part psychological? Like, maybe your stress level was so high (physically and psychologically) that something that used to work didn't?
The only thing I can think of that I've experienced that's similar involves benzos. Sometimes, .5mgs Klonopin was fine. Other times, even when I wasn't benzo-tolerant, I needed a full 1mgs to get 'er done. It could've been psychological, too many stress hormones coursing through my veins, too much caffeine...who knows, really?
The important thing for you (and for me, too) is that you keep your psychosis under control most of the time. It sounds like you're being told to use the neuroleptics in part for agitation and anxiety, which is OK, but that's not their main indication when used long-term. The big picture is psychosis-control. I know with my neuroleptic (Abilify), I had to wait weeks before I stopped freaking out and hearing voices. Short-term, that sucked, but long-term, I've been able to keep things under control w/ no major problems.
Can you take a benzo? You can only push antipsychotic dosage so high before you start running into problems, and it seems to me that if you need additional sedation now and then a benzo shouldn't be a big deal. They're also quite cheap, since they're all in generic form now (except for Klonopin wafers, I think). Just a thought.

I think C_E is right in what he has to say. The term for what he describes is "medication breakthrough". It is often the result of an increase in psychosocial or physical stress. However, some people experience a relapse or worsening of depression in the absence of stressors. It can be the product of the natural course of the illness. The healthier you are psychologically, the less chance there is for medication breakthrough to occur. This is where one can glean benefits from psychotherapy.

So what do you do when you are med tolerant and just want to sleep and don't seem to need the meds but are a total scared wreck during the day due to life circumstances and the frozen deer syndrome. Afraid of your shadow? Too many things in life changed all at once. Zyprexa sounds like if for two days only something was going on to upset you? Phillipa

Well I don't know how to explain it but everything is good now! The only change I made was dividing up the 15mg dose into 3 x5mg doses, that I take at the same time as the 3 x8mg doses of perphenazine. I'm not forgetfull at work anymore. No holucinations, no anxiety. I feel so good like when on 10mg zyprexa before all the med trials. But that was once a day. And now it does not even seem like I'm taking perphenazine, but I am. Its like the zyprexa is working again! How do I explain all this to my doc, so she does not decide the zyprexa is not working and wants anouther battery of med trials. I wonder if I even need the perphenazine and bupropion? What do you think?

My advice at this juncture is that you not "rock the boat". Leave things alone for the first few months. Don't add nor subtract drugs. If you want, you can try tapering one drug at a time later on.

I'm wondering if I should increase the perphenazine back to where it was. And probably the bupropion sr as well?
