I have been taking Parnate 30 mg/day for about 6 weeks and was out with friends and consumed some queso (cheese) dip at a Mexican restaurant. It wasn't on purpose but I just had a momentary lapse of remembering I can't eat cheese. I probably had about 20 chips w/ cheese dip, and then realized what I had done. That was about 45 minutes ago.
Here's my question: If someone has a reaction to tyramine, how long does it take for blood pressure to go up? My BP is fine so far, and I will go to the ER immediately if it rises beyond normal, but its registering normal values. Will I be in the clear, say, 90 minutes after eating the cheese, or is it possible that I won't have the symptoms until later? Thanks.

Its been nearly 2 hours now and my BP is still around 120/80 and I have no symptoms so I guess I was just freaking out a bit. After I posted this thread, I searched this site and it seems that most people start to react 30 mins-1 hour after eating the food. I'll take my BP a couple more times but I'm sure I'll be ok.
Admins can feel free to close this thread, or leave it open if anyone wants to describe any of their experiences with MAOIs and accidentally eating the wrong foods...Its very hard to find much info on the time frame of a hypertensive reaction to MAOIs and my doctor unfortunately isn't well versed on MAOIs to say the least. I'm glad Dr. Bob's site is here to provide info of this sort. Thanks all!

I'm glad you asked the question. I've done that before and wondered what the "danger zone" time frame is. So is it 30 min-1 hour after eating the food? Are you pretty much in the clear after that?

I think the effect is almost immediate. When I started on parnate, my p-doc said that very few people prescribe MAOIs anymore. I responded that, for someone who is suicidal, like me, you could eat some salami and cheese with a bottle of chianti and check out. He said that's not true. These are tendencies, not certainties.
I have found that I can have small amounts of cheese (parmesan on pasta, blue or feta in salads) with no ill effects at all.

When I had a hypertensive crisis I would say I started feeling it an hour after I ate the monterey jack cheese (unmelted - I have eaten melted before without problem). It started off with a bad headache. My blood pressure was high. I freaked out. I bit into and swallowed a nifedipine capsule and took a taxi to the ER. My BP shot up to something SCARILY high. I went to the counter totally in pain and the first thing they want is the $100 co-pay. Are you f***ing kidding me?? There was no one in the ER but for some reason I had to wait for a long time with this horrible headache. Eventually, with just the nifedipine I took myself, the BP came down gradually. A few hours later (4am) I walked back home and went to sleep. I will never eat monterey jack cheese, unmelted, again.
And what's with pdoc's saying no one really prescribes MAOIs anymore? On this board alone I've seen a lot of postings about them. I know at least at my teaching hospital the residents are at the least aware of MAOIs as a viable option. Risks aside, MAOIs can help when nothing else has worked or only has worked so much or has stopped working.

I have gotten away with accidentally eating cheese several times. However, I still am scared to eat it on purpose. Do you think maybe it didn't matter if it was melted or unmelted but rather it's all Russian roulette and then same cheese you got away with eating last Saturday can make your BP skyrocket this Saturday? That's my theory. I ate a huge plate of cheddar cheese fries and my BP actually went down. There's no way you could pay me to eat it again on purpose. I only tempt fate with a good reason and cheese fries don't qualify!

Maybe the reaction is more likely to be within an hour and a half, but it doesn't have to happen within any specific time frame.. It can potentially take much longer, so while it might not be necessary to monitor so carefully after a couple of hours, you probably should be vigilant to some degree afterwards. I wouldn't ignore anything unusual the same evening, and would probably check a few more times.

Aren't there also spontaneous hypertensive crises that seem to be from nothing? Phillipa

Just to update, I didn't have any reaction and my BP was normal. Thanks for the input, this site has so much info about MAOIs that are hard to find.
This is the end of the thread.
