I have had panic attacks since I took Sulpiride, I got fever and was afraid it was NMS. I took ibuprofen and the fever settled and I have no muscle rigidity. Are my concerns irrational? What are the risks of getting NMS with sulpiride at low dose? You who are experienced AP users and other knowledgable people, who does one tackle such anxiety?

I was just looking at this page yesterday actually. I'm sure I heard something crunch in the first video:

Just imagine all the horror stories War On Drugs scare-mongerers could generate with video clips like that... NMS is so rare you'd be *very* unlucky to get it from low dose sulpiride though. Sulprirde isn't asociated with especially high rates of NMS as far as I know. In fact, I would have thought low doses of sulpiride would be particularly unlikely to cause NMS as it's thought to be due to excess D2 blockade, and sulpride has almost the oppposite effect at the doses you're taking. I had a bit of a temperature on Zyprexa though and wondered the same thing, but nothing came of it.

To me 50mg would be a high dose to start with, though not so with others probably. I'm sure the dopamine kick would give me panic or anxiety at that dose right at the start. When I took amisulpride I started at 12.5 mg for a couple days, then up to 25mg, then 37.5mg, then 50mg. My best dose was 25mg.
I doubt NMS is a concern. It is possible with any psych drug, especially ones that work on dopamine, but it is rare. My best guess is that it was too much of a dose too fast.
The dopamine kick should fade in a few days, seems to poopout sort of, and then in a couple or three weeks the true therapeutic effect takes over. In the meantime, you might want to back down on the dose and feel it out. Lots of people friek out when they start Lexapro 10mg or Effexor 37.5mg or Prozac 10mg or whatever. Even though those doses are tolerated ok by a lot of folks, they are just way too much for others. It takes time for some people to get acclimated to a normal dose.

As compared to side effect, and the lower concerns of TD, NMS is ver very rarel\.Its interesting you responded to ibuprofen for fever -- generally only scentsminophe/paracetamool anf possibly aspirin itself eorks for me.

But anyow, the feeling of fever or even the fever itself are things that can cause and exacerbate fever/
Nothing is irrational there, its just hypervigilance. You know you have panic attacks, everyone gets around 5 colds or fevers a year (or less) an infectious specialist told me once, and you've read up on NMS.Having said that, lots of other nasty things like ataxia, confusion, high (103-104) fevers, would surely follow.. then it is get thee to an ER;
No a lot of things, including medication lower WBC counts, but I wouldf file it as I know it, I red it, I have a cold or virus.

Some drugs do interfere with the body's thermostat, particularly TCAs and Effexor. I had hot flushes and cold sweats on those - many people do. I also developed 'pyrexia'during the first few days on escitalopram, and it's listed as a side effect in the patient information leaflet. It's possible antipsychotic drugs can affect body temperature (in a benign way) too.

Well, my mother tongue is swedish so I'm not the one to judge your grammar. The important thing is your message, not how well you spell it. Anyway, I quit sulpiride, I'm not ready yet for APs, how ridiculous it may sound but I want to try more hands-on approaches like modafinil. Will meet my pdoc at monday and discuss this further.
