Posted by clubfitter on November 29, 2007, at 20:05:27

Im very desperate here and need something to work ASAP. I've heard MAOI's work pretty fast? Is this true? Im going to talk about it with my doc. What have you tried that has worked fast? Thanks!

Adrafinl. 3 hours.
Modafinil. 3 hours.
Prozac. 2 days.
Ritalin. 1 hour.
Marijuana. 15 minutes.

All led to later problems or pooped out, but that's how fast they worked when I started them. In a pinch I will not hesitate to pull out a tab Ritalin to save the day when the dark clouds are just too heavy to bear any longer. Thankfully that hasn't happened in months, though close almost every day.

Lexapro was the fastest for me but pooped out like all SSRI's do. Currently on Nardil..took 13 days to kick in and I'm pretty impressed with it thus far..

Having loaded up my nervous system gradually on 90mg per day of Nardil I'm trying to see how low I can go without recurrence of symptoms. One evening I'd obviously gone too low. Like flicking a switch I felt terrible again. So I immediately took 30mg of extra Nardil, 25 mg Nortriptyline and 300mg Niacin (I find the flush calming). Within two hours I was feeling great again. It's just that to get to this level, when topping up works in my experience, you have to build up the dose and wait. In my case 6.5 weeks. It was worth the wait. My depression and anxiety was replaced by euphoria, presumably a side effect. This lasted 2 or 3 weeks then I felt normal.

I concur with everyone who says - MAOIs should be first-line treatment for depression

I had exactly the same experience with Mocloemide. It was GREAT for 4-5 days and then no effect. That's why I'm hoping to get on a real MAOI to see if I get the same effect, but sustained over time.

Zyprexa the first time around at 5mg. After 3 days I had no use for Klonopin and I felt wonderful. Unfortunately it did cause shaky hands and I was put off by that. An atypical will stop depression in a very short course and long term transition to a fairly calming SSRI such as Luvox or Paxil which would cover anxiety and depression would be one idea.

i think my saving grace medication has ALways been zyprexa...it is fast acting for me...literally within 24 hours i feel a difference.
(ex. this last time i went to the hospital, when i was admitted i literally could not physically stop crying except for a few minutes of reprieve here and there) within 24 hours i stopped...maybe partly because of hospital environment, maybe partly physical exhaustion....but quite frankly i've been on it before...i know its the med.)

and also i agree about the ritalin. except that its very short acting. only helps for a few hours...better than nothing and VERY helpful if you are in a danger zone.

********
do becareful if you have ever had manic reactions to meds.
i have expecially to effexor and prozac (but don't let my responses to meds effect you....its just a cautionary note) one of those meds may work Wonders for you...

just alwyas be alert for a change in your normal behavior.

best wishes
b2c.

don't mean to hijack thread but have a question for Justherself54...
my pdoc wants me to start lexapro.
i finally got the nerve to take one this morning.
-how soon do you notice a difference?
-and do you have MDD or BP? if you have BP did you have any manic issues with this med?
-and how long before it pooped out...

sorry for the 20 questions...but i appreciate any input you can give...
Thanks!!

Hi

I started on 20 mg and ended up on 80 mg. After 3 days on 20 mg I could already feel the effect. I thought I was imagining it because no AD works that fast. It was acting as a stimulant. So my mood didn't lift in 3 days, but with the help of the stimulant effect I did feel better and was able to accomplish things I hadn't been about to in a while. My mood started to lift after 3 days on 30 mg.

I have heard others say the same thing about Parnate (more than with Nardil) so I am not the only one.

I hope this helps!

Maxime

Hi B2c. I noticed a difference within 4-5 days..I have BP. I did experience a mild euphoria for a day or two, but nothing that concerned me..it pooped out on me after a year..that's why I'm now trying Nardil..hoping to find something that doesn't poop out after roughly the year mark..Lexapro was great for my social anxiety and I was even able to return to work until I crashed after the poop out..hope it works well for you as it did for me initially..don't remember any start-up side effects either..I found it a good med, next to Zoloft..except I suffer from the dreaded poop out syndrome! Good luck to you..keep up posted on how you're doing..

Mirapex. Adderall and ritalin have faster onset (immediate, significant increase in dopaminergic transmission) but have nasty long-term effects and are thus usually not sustainable.

A better approach is: What is the specific nature of your symptoms, and what underlying causes do they point to? (Serotonergic dysfunction? Dopaminergic? A combination of the two? Something else entirely??)

It's best try to treat the specific causes. Hard part is figuring out what those specific causes are. Only good ways I can think of are MRI, PET, and deductive analysis based on descriptive symptomology. For example, anxious, fearful depression points to serotonin; apathetic, anhedonic anergia points to dopamine. Overly simplistic analysis, to be sure, but better than the one-size-fits-all shot-in-the-dark approach to prescribing meds that most clinical psychiatrists rely so heavily on.

Ever try a 5-HT2a/c antagonist like Mirtazapine? It's no wonderdrug, but its 2a/2c antagonism can be fast and very effective for driving away that constant irrational, fearful state.

Can you believe I was in the hospital and they put me on l5mg of remeron with chloral hydrate and started the valium so some xanax too. And I didn't sleep up in four hours of also 50mg of seroquel which when woke-up they repeated and I almost fainted when woke up again. End of the seroquel. Stayed on the remeron for a month with the others. Nothing no side effects just nothing. Phillipa ps must have tx resistant sleep.

Fastest acting? My picks.

1. Provigil (Can cause stimulant low at initial stage. No crashes after repeated dosing)

2. Stablon (Has to take 25mg TID. Can be reduced to the standard 12.5mg TID after a week or so)

3. Trivastal Retard (Notable effect after two to three days)

4. Solian (Dramatic early effects soon diminish. But subtle AD effects persist)

5. Dogmatil (Same as Solian)

So you don't know how the Remeron would work by itself since you were on a combo of medications.

Remeron dose ranges from 15-45 for usual dosing. You perhaps could try this again alone and see what happens.

I have noticed that many of the med combinations I have taken do not give me a good idea of how any of the meds would work on their own.

I failed Marplan but I went back to my notes and saw I was on several other medications with it. In order to know whether it would really work again I would have to take it by itself.

Perhaps remeron would work for you alone, or perhaps just with the Valium, you have taken for so long, since I don't think you can come off of that without a real desire to do so and a plan to wean off it. But you really haven't given Remeron a good trial since you only took 15 mg and the other meds you took it with might have prevented it from working. There were too many ingredients in that recipe to know what was doing what.

They call that a shot gun approach. No real theory behind it, just using alot of meds to try and get any response.

Stargazer the answer to that one is a pdoc put me on wellbutrin 150mg for a week and saw the pdoc and he thought I was manic. So sent to inpatient and they decided to try and make me sleep but since my first child I've had the same pattern of waking after four hours did it then to make sure she was still breathing. I was only l9 then. Plan at the hospital was a taper off the chloral hyrate and xanax and go on the valium first time in years. Think they gave me zoloft for depression. Phillipa

You missed my point of the story. If you tried Remeron alone the effects may work completely differently than when you are on 3-4 other meds.
Taking many meds together may not work the same as when you are only taking one med by itself, say Remeron.

I know but is it an option (Remeron) you would try since you have such fear in trying new drugs. Will you have to stop any of your meds before surgery?

The only way someone will get better is by trial and error unless you are content with the place you are at. There's nothing worse in my book than taking something for which there is no improvement in my functioning.

Why take anything if it is not giving you some benefits, that makes no sense to me. All meds have side effects so you have to make sure your choice of meds has benefits to it even though the SE's may seem minimal, you are still affecting biochemical reactions in your body that over the long term might cause other problems.

hope your surgery goes well, is it scheduled yet?

Stargazer
