Ritalin didn't work out.

Tried adding ritalin to prozac/zyprexa. It was just like tyrosine. That is, excellent response with the first dose of the day...then depression worsening as the dose wears off...then an improvement again after the second dose but not as good as the first dose...then depression worsening even more as the second dose wears off...then no benefit at all from the third dose...incredible despair crying suicidal depression as the third dose wears off.

Scary stuff for me. One day of that was one day too much.

I have had the same reaction on ritalin, but the depression without it is just unbearable. Doctor tried to switch me to Concerta, but it has little effect. I'm thinking about suggesting Adderal. I've heard it lasts a little longer than ritalin and doesn't have as hard of a crash. Right now I'm in a quandry. Zoloft isn't working, but scared to try new medicine (Effexor). Had a massive panic attack today that led me to a dangerous level. Maybe the doc should up my Zoloft. I frankly don't know what to do anymore. Life sometimes feels unbearable.

What's the Ritalin for? If it's to augment your AD, have you tried or has your doc suggested wellbutrin? I hate bringing it up because it worked so poorly for me, but since ritalin/focalin work so well for me maybe you'll opposite response to it than I did. Wellbutrin and Ritalin use different mechanisms AFAIK to block the reuptake of dopamine, which may have made all the difference for me (and why it didn't work for you).

My previous PDoc, about 8 years ago or so when I was first starting on multidrug therapies, told me that there was a lot of support in the literature for the effectiveness of prozac and wellbutrin. With zyprexa as a mood stabilizer, that might be a combo to discuss with your doc.

Wellbutrin is an approved antidepressant wherase ritalin is not. Wellbutrin isnt all that strong a dopamine uptake inhibitor, its mechanisms as an antidepresant are more complex.

I havn't heard of crashing as a side effect.

I think the more technical name for the crash is "rebound" effect. Ritalin is great for the two-three hours it works for me, but then I plummett and my depression becomes exaserbated. I also deal with the occasional panic attacks. I've had these for a while and don't attribute them to the ritalin. I take 1mg of Klonopin for these, but it leaves me down and with a big headache. My mission now is to try and find an antidepressant that is going to work. I hate the trial and error game. It's a toss up...do I want to stay miserable or gamble that I will find a medicine that will help or God forbid make it worse.

Yeah, doctors try to rephrase things like that. A rebound in innatention would be expected, but the crash is an emotional crash too.
