I am so frustrated. I saw my OB/GYN and he said I could switch to different bc pills. He also wants me to take Sarafem (Prozac 20mg) during the 7-10 days I have bad PMDD every month. I told him I had taken Effexor in the past and it worked well for me, but he said to try the Prozac first. He said it would help with the bloating and pain.
My shrink knows I could get manic on SSRIs and she says go back to what we know works which is Effexor. She also swtiched the Wellbutrin (since it makes me irritable) to Provigil to counteract the sleepines of the Effexor.
Who am I supposed to believe?!

I would definitely go with the Pdoc! Yes, you might want to switch to a different BC pill, but I would even run that by the pdoc! The Prozac did nothing for my daughter for PMDD.

B6 works for bloating my OB-GYN told me to take it years ago before all these meds were out. Love Phillipa

In matters relating to my mental state and meds, I'd definitely trust my pdoc, esp. if she has a track record and has figured things out. Sounds like SSRI's are contraindicated, and your Obgyn may not understand the full implications.

What your pdoc and your gyn are trying to do are totally different things, so likely they're both right about what they're looking at.
Your pdoc is considering your mental health, your depression, etc. That's great, and that's what you want, and that's the best place to go for mental health issues.
BUT
Your gyn is talking about PMDD, which is different. The SSRIs have been shown to be very successful for treating PMDD, if taken for three or four days around the worst of your symptoms. They're not treating depression, since they work in the few days you take them every month. They're treating something else, which they're apparently very good at treating, even in the few days they're taken. I think, though, that Paxil showed as working better in studies, but that's neither here nor there.
So, who do you listen to? I can't answer that. I can only tell you that your doctors are both talking about their own spheres of expertise.

I know you are right. But, in the past SSRIs have made me manic over time - even though they help the PMS, depression & mood.
So, I guess I will stick with the Effexor for now. My doc said that since the Wellbutrin makes me ittitable (even though it works well for my ADD), she gave me Provigil to take if I need it.
But, so far so good on the Effexor.

No Brainer (for me anyway) the pdoc "should" know a lot more than the GP in this particular area. Go with the Pdoc's advice.Monte

I'd expect a pdoc to be familiar with use of sarafem for PMDD, so I'd include that w/in her area of expertise--whereas an obgyn would necessarily be as expert about use of other pmeds, esp. those that might work better for you.
I still think your pdoc is probably more on top of this particular problem.

Hi Trish~
In having worked for an Ob/Gyn for 8 years, I can tell you that they do often prescribe meds. such as SaraFem for PMS, and other A.D.s for Menopause.
In my honest opinion...NOBODY but a Psych. should be prescribing meds. such as Anti-Depressants or Benzos.,including SeraFem.. but they do. I can tell you that when I was with this particular office, Zoloft was the biggie...and I am not kidding when I say it seemed our entire patient population was on it...unmonitered...just given samples and a script and sent on their way.
Now, these are a really great bunch of all female Dr.'s..Actually, they are my Gyn Docs., to this day..and they are great GYN's, whom I love to pieces...But they're NOT Psychiatrists!
I personally am partial to Effexor..only because it was the med. that worked for me. I BEGGED for Wellbutrin first..because I was scared of weight gain. Well..it was a disaster...and my "Psych" quickly switched me to Paxil...another disaster. FINALLY, to Effexor with Xanax...and ding, ding, ding!!! We had HUGE success! Now...there is NO way that an OB/GYN could or would be able to handle all of that, and give the follow up needed to moniter the meds.
Do yourself a favor...Treat each problem with the appropriate Specialist. *A Psych can't handle a Gyn issue, anymore than a Gyn can handle a Psych issue*....Listen to your Psych...and yourself, Trish...and let he or she help you to figure out what's best...Not your gyn.

Either could give the better advice. I would choose the one who seems the most intelligent, well-read and conscientious.
