I've been on remeron since July (my second go round) and have recently been having heavy duty heart palpitations along with extreme sleepiness during the day. I have gained about 30 lbs on this med.
Does anyone out there have medical knowledge about remeron having caused heart problems? I will call my doc, but it takes a few days to get an answer from his nurse, so thought I'd try to get some wise info here.

This is probably a phenomenon known as orthostatic hypotension. This is caused by a blockade of the alpha-1 adrenergic receptor, which is a target that Remeron binds too after it finds its way too the alpha-2 receptors which mediate its proposed anti-depressant effect. Blockade of alpha-1 receptor causes a decrease in blood pressure which in turn causes the heart to beat harder and quicker to get more blood through the body. Do you find that this happens alot when you stand up or lay down alot? I too take 30mgs Remeron (at night of course) and also find I get this occassionally. It will not hurt you, but you should report it to your doctor next time you see him/her.
I hope this helps you,
Justin

Actually, yes, this did happen after I'd worked very strenuously for about 3 hours. When I got home I couldn't stay awake and slept for about an hour. When I woke is when I noticed the heart palpitations.
I will mention it to my doc and thankyou for the information.
Best wishes,
