After about 6 months of being treated for depression, I finally
got a dx of ADD last month and am in the process of having my
meds readjusted. Up until 2 wks ago I was on 75 mg of
Zoloft, 0.5 - 0.75mg of Alzam (anti-anxiety, I think this is
a local brandname) and 10mg of Stilnox (another local brandname,
equivalent to Ambien I believe). I've now dropped the
anti-anxiety drug, and am on 25mg of Ritalin a day (3 doses of
10 + 10 + 5), with 50mg of Zoloft and the nightly sleeping pill.The Ritalin has *definitely* cleared away the "white noise" in
my head and helps me focus and achieve things in the course of a
working day. But at the same time I feel unbelievably
irritable- like my worst dose of PMS x10 - and I can't stop
myself from being aggressive towards and picking fights with
colleagues and family members. It feels as though it's
escalating this week and today's been particularly bad, winding
up with me getting into a raging argument with someone, then
crying all over them. This is NOT me and no it's NOT PMS, and
I'm really worried that it's just going to get worse. My
doctor's going on vacation tomorrow and my next appt with my
therapist is on Monday.. I hate calling them "just because" and
honestly I don't feel that my meds have been very well managed
by them. Has anyone else had a similar experience? Or does
anyone have any ideas? Could this be the result of cutting down
the Zoloft?

Most adults HATE ritalin because of the way it makes them feel:
irritable to say the least, especially as the dose is wearing off,
which in the case of ritalin, can be as soon as 1 hour after ingestion.The vast majority of adults that I've met or communicated with prefer
adderall, a mixed amphetamine sulfate. It lasts 2x as long as ritalin
in some cases and there is no irritability from the crash. Dexedrine
might also be a possibility for you.One other thing you might address with your pdoc is interaction with
zoloft. I know the PDR says that SSRI meds may dampen the effect of stimulants
but my own experience with paxil was just the opposite -- it potentiated the
adderall instead.
Now I happily [and calmly] take a TCA with adderall.That said, I would be very hesitant to change AD meds: I've seen them not work
a second time too often.And remember, getting the dosage of a pstim correct is a long process of trial and
error. The results however, are worth it.
