insurance: disability and long-term care

Hi:

Does anyone know about disability and long_term care
insurance for mental conditions. My disability policy only cover for two years, unless I go into an
institution. Are there others out there that cover for
longer than two years. The only other option is social security disability which doesn't pay much.
Also is there insurance that would cover a person if he had to hospitalized or in a home for a long period.
