I think the documented doses are just sometimes too high. I saw a case report where 15mg eliminated a woman's depression. Me, a mere 18mg had me lifting out of the pits in about 10 days, actually beginning the second day.
For urinary side effects, the prostate med Flomax (Tumsulosin), works like a charm. It comes in 2mg and 4mg sizes, but I found taking half a capsule was just fine. Other side effects can be tough to handle, but users say they go away in about a month.
You can get empty gel caps, divide up the milnacipran powder into fairly equal piles of custom sized doses, reload them into your gel caps, and make your own doses. The best for me was 6mg 3 times per day.
It is the best med I've ever tried since ECT failed. That says a lot. Any med that can show promise after ECT failure has got to be a good med.
Just go slow and low. Forget the dosing instructions. Do your own.
