switching from lexapro to zoloft well on wellbutri

My doctor is switching me from lexapro to zoloft. How do I wean myself from lexapro and then start zoloft? How many days from when i stop lexapro do i wait to start zoloft. I am also taken wellbutrin 3 times a day. Thanks for any advice you all can give.
