How can you tell if you have a chemical imbalance?
You may have feelings or behaviors unacceptable to yourself or others. You may have recurrent depression, anxiety, or a decrease in your level of functioning that cannot be explained by traditional methods such as psychotherapy or physical tests. You don't feel right, reality doesn't explain the feelings, and it keeps happening. Blood testing is usually a part of initial evaluation, but usually more is learned from family history and reactions to past medications. Blood tests will not usually detect a brain chemical imbalance. If you have a genetic background, you are not at fault. You inherited it. You can fix it. Your ancestors had no choice. You do.

If Nortriptyline was working, why stop? If it wasn't working, then there are plenty of other good medications to choose from. As one example, Tegretol could be an excellent substitute for both back pain and mood swings. Your mileage will vary, as with any medication. Everyone's chemical imbalance is unique. No two are the same.
