Why antidepressants stop working for some people?

I was talking to a friend last night about her long
history of having taken SSRIs which work for a while
and then stop working. Also, not all SSRIs work, and
she has a tough time while figuring out what the next
drug should be. Now that we understand better why
and how these things work, have there been any
discoveries about why they fizzle out for some people?
Are there non-psychiatric health problems which would
cause this effect?
